How to apply 

Making an application

  • To get an application form call us on 0800 328 1159 and we will send you out a form.  If you would like to download and print the form yourself you can do so at apply for a grant, where you will find the form in both word and PDF formats for printing.
  • Complete Section 1 of the application form.
  • If you are applying for a computer complete Section 2 of the application form (Computer Form Part 1) and pass Section 3 (Computer Form Part 2) to someone at your child's school who knows what computer equipment they use there.
  • If you are applying for Speech and Language Therapy pass Section 4 of the application form to someone at your child's school who knows about their speech and language difficulties.
  • Contact the company who will provide the equipment or service you're applying for and ask them for a written quote. If you are applying for a computer or trampoline you don't have to do this.
  • Get a medical reference - we need you to get a letter written by a medical professional involved in your child's care confirming their condition and that what you're applying for will be of benefit to them.
  • Get a second reference from another professional working with your child, this does not have to be from a medical professional but needs to confirm the same information as the medical reference.  If you're applying for a computer Computer or Speech and Language Therapy you don't need to do this as the form you give to the school acts as a second reference.
  • Provide financial paperwork - we need a copy of a complete recent month's bank statement from your main bank account and copies of your and/or your partner's payslips if you're employed.  If you're self-employed we need information on what the business does and your income and expenditure for the last financial year.
  • Check the ticklist on the Section 1 of the form to make sure you've included everything.
  • Read and agree to the declaration and sign the form.
  • We suggest you make a copy of the form and all the extra information for your records and in case of any problems with the post.
  • Send the form, references and financial documentation to Cerebra Head Office, the address can be found on the front of the form and on our Contact Us page.

It might seem like we ask for a lot but we do need all of this information to make sure we help the people who most need it.

Important: If anything is missing or your form is not signed it will delay your application.

What happens next?

  • When we receive your application we will contact to let you know we've had it
  • If there is any information missing we will write to you to ask you for it.  Sometimes these 2 steps are combined and the first thing you'll hear from us will be to ask you for missing documents.
  • Once we have all the information we need your application will go to a panel for a decision.

What happens after my application is approved?

  • We will write to you to let you know your application has been approved including a copy of the quote you sent us with your application and a slip for you to return to us accepting the grant.
  • You need to check the details on the quote, then complete and sign the form to say that you still want the grant and return it to us.
  • We also ask that you send your contribution towards the cost of your grant at this point.
  • We will then order the equipment from the quote and the company will deliver it to you but invoice us.
  • In a few randomly selected cases we have to carry out a home visit before the grant can be offered.  We will let you know if this is the case for you as soon as possible after your application is complete.
